Changes include: Making the first paragraph simpler and more direct Dividing the “follow-up instructions” into a set of three clear steps for the doctor and the remaining items for the family Removing the item for getting an inspection (too expensive to be realistic) Adding potential links to social media There was aiso a suggestion of providing an example of a completed form, especially for the “Local Resources” section. I'll work with one of our local partners on that.
