Hello all,
Your quarterly report for the 4* quarter (July-September) of FY14 is due on October 30. For your convenience, I’ve
attached a copy of the QR form for Prevention.
PLEASE NOTE: You are released from any obligation for FY14 regarding the “Community Prescription,” so there is no
need to report on that activity this quarter. Development of this document has been a long and arduous process, with a
variety of views—all of them with merit—on the best language and format. In the new fiscal year, we’ll work together
to get it into the hands of providers and the families they serve.
