Forms for Potter Elementary

Kirby — | believe | forwarded you this form previously (on 11/20/15), but a decision was made to only do one school that weekend. As a result, you may already have some forms printed for Potter Elementary. Potter Elementary is the only school planned for sampling this weekend (12/5/15). Ad this time, we do not know how many sampling tap locations there willbe. As a start, let’s assume we need 150 small {125 mi} bottles for each school: and 30 large (1 Hter} bottles for each school. As you know, we'l need at least 146 forms printed far each school We should also cortinue to receive an extra box of each set of bottles fas you suggested last week). Thanks, Liane. We'll have those bottles and forms ready for tomorrow afternoon. | wonder if it might be helpful to give you guys a spare box of each of the smaller and larger bottles, so that if your estimates are off slightly you can tap into these reserve bottles as needed. Also wondering if you all could start thinking about the upcoming holidays and any sampling plans around those days as staff are typically beginning to schedule holiday time off now. thanks! Kirby

Sample forms

Attached are two forms for this weekend sampling: Central Kitchen and Durant-Tuuri-Mott Elementary. For Central Kitchen — we estimate we’ll need about 100 32CC bottles and 30 36CC bottles. For Eisenhower Elementary — we'll need 180 32CC bottles and 50 36CC bottles. Steve is planning to call me after they complete the next assessment so we can readjust if necessary. Kiray~ Attached are new forms for Brownell and Elsenhower. We'll need 180 of each (150 32CC bottles and 30 36CC bottles} for each school. (2 sets of 180 bottles). We'll also need 20 request for analysis forms for Freeman Elementary along with 30 36CC bottles. HW there’s a check box for S2CC or 36CC, then staff won't have to be careful about which forrn they pick up. That makes the most sense to me, but ’m anen to options.
